We're honored that you've allowed us to become your family doctor. To help make the most of your first visit with us, we'd like to collect some information first.

Your demographics and insurance

To help shorten your waiting time in our office, please print out and complete the following forms and bring them with you to your appointment. This will help us ensure that your file is accurate and up to date.

Your medical history

If you have seen any other doctors within the last 5-7 years, or have had any surgery or health-problems in the past, we need to review your previous health records. This will allow us to continue your existing care in the smoothest and safest manner.

If you can arrange for those records to be sent to our office well before your visit, the doctor will be more familiar with your health history by time your arrive.

To help you retrieve your records, we have prepared a form that you may download and submit to your previous doctors (by mail or fax). This will help them reach our office to send your records.
